Eradicating DNS Rebinding with the Extended Same-Origin Policy

The HTML5 Offline AppCache lets an attacker keep script cached until the browser's DNS pinning lapses, making DNS rebinding reliable in every major browser; a two-domain variant survives cache revalidation and repeats. Demonstrated against Polipo and CUPS. The fix adds a server-declared X-Server-Origin to the origin check, implemented in Chromium in 34 lines.
